What is tested — the official NCSBN blueprint
The NCLEX-RN is organised into four Client Needs categories, two of which are divided into subcategories — six subcategories in total. Every scored item comes from one of the eight rows below, in the proportions NCSBN publishes. NCSBN also notes that an individual exam’s distribution may vary by up to ±3% in each category.

Frequently asked questions
Is NCLEX-RN scored all-or-nothing on Select All That Apply (SATA) items?
No. NCSBN states, in the 2026 NCLEX-RN Test Plan and in its candidate FAQ, that "there is partial credit scoring for items for which more than one key exists", and names three methods: plus/minus, zero/one and rationale scoring. NCSBN's own help centre confirms that Select All That Apply items specifically use the plus/minus method, replacing the older all-correct-or-all-incorrect approach. NCSBN has not published the exact formula, so CARE Nursing's practice-mode grading is an honest exact-match simplification rather than a claimed replication of it.
Does NCLEX-RN include Next Generation NCLEX (NGN) case studies?
Yes. The 2026 NCLEX-RN Test Plan describes the NCSBN Clinical Judgment Measurement Model (NCJMM) case-study architecture: a minimum-length RN exam includes 18 scored case-study items (three six-item sets, one item per NCJMM step — recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action, evaluate outcomes) plus roughly 10% additional stand-alone clinical-judgment items. NCSBN's own sample exam preview pack includes 3 RN case studies.
Did the NCLEX-RN blueprint change for 2026?
The 4-category / 8-subcategory structure and percentage ranges are unchanged from the 2023 Test Plan. One subcategory was renamed: "Safety and Infection Control" became "Safety and Infection Prevention and Control" in the 2026 Test Plan.
